What would you base your MDH on for the NDB? Because of course, you wouldn't fly an NDB approach without having an MDH otherwise you wouldn't know when...

I am disagreeing with your view that the 200' recommendation only applies to the MDH for straight in approaches.

It is a recommendation to apply to ALL MDHs.
