Its not easy to tell what changes. I would say try and buy the latest due to changes in EASA and GPS use in the past few years. I dont think they've released a UK-FCL version yet.

I agree with Markos, ICAO Pans-Ops 8168 Vol 1 defines an approach as starting at the IAF. Without any other instructions. You commence it there unless cleared to start it at the IF.

Depends on the authority. The UK CAA did it in 2019 for 727 and DHC-7, those sims were in North America. Lots of paperwork and safety cases involved.
