Call me old-fashioned, but when I call for start-up clearance and ATC say "clear to start", I reckon that's clearance to start the engines.
Whether it has any effect on my Flight Plan, I've no idea.

For Cat D it's 200m down to 150m with same conditions.
787 with HUDs can do 75m with approvals.

It's because the Manoeuvering speed is different to the Approach Reference speed (Vref) which is what your FMC page is showing.

In the Boeing Procedure turning the APU off is the 1st item after
engine start. It then goes into cooldown mode. I'll check how long
that lasts, but not long. I think.
