Papi's are not required, nit even on an NPA. There are runways without them and sometimes they are notam'ed US.

What are the missed approach procedures while on a visual approach following vectors

No reference, but as you were initially vectored for an ILS, thats what ATC expects. So fly that GA.
When ATIS states "ILS rwy xxx" it's the same for me.
Now, if ATIS/approach only states "rwy xx"...

It says and/OR. So, if an alternate (or two) is/are needed you are only allowed TO when that alternates meets its planning minima.

Vref headwind correction on a CJ2?

Miles, of course you calculate the LDR based on your expected groundspeed, or, with the increment added to Vref. No added risk for overshoot.

Good question on the need to do it for light(er)...

Well, our books say ETP=V2, so yes, I would pitch ip to V2 if above.
Its not up to me to decide if its a gnats cock or not.
Was it concluded they would have survived if they maintained speed?

Officially its airborne exactly at CTOT. The -5/+10 is to accomodate taxi delays. Fly to Sweden and some Spanish airports and you'll know...

I did it in the SIM so don't know for sure, but it's a licence upgrade, not a rank upgrade. You're still an FO, so right seat.

The danger with the Boeing procedure (setting the highest SID alt in the MCP) is if you are high perfomance due whatever reason, you might have to revert to VS mode to reduce your climb rate to below...

Am I right to worry about an airline that does not carry enough fuel to hold?

Was the fog forecasted? If not, tough luck. If yes, was it below approach ban minima? If yes, tough luck. If not, ask airline to explain.

Normally when you aren't able to perform an autoland anymore with the B737 you gonna switch to an fail-passive automatic landing system.

Because in EASA-OPS levelling off at the MDA is not allowed. Only CDFA. (There are exceptions though).
