Personally, No I don't think these are grounds for a fail.
Mistake 1, the PF would normally ask you to switch NAV frequency's, these things happen. Its how you both deal with the mistake.
Mistake 2, I would have thought you making a go around due to not being happy that you are stable would be the right decision, as a trainer I would be more concerned if you chose to carry on if you are unstable. Again its how you handle the go around and deal with it.
Of course these are my own views as a TRE, have you asked for feedback where you went wrong?

Hey, does anyone know the rough timeframe between submitting the online assessment and then if passing how long after they would expect you in Dublin for the interview/sim assessment?

Hello everyone, i got my assessment on AFA HQ on November 7th.
I will book some sim hours on Barcelona (Hangar 10) or Valencia (flysimNG).
Hangar 10 is 100€/H
FlySimNG 90€/H and discounts if you do 5 or 10H pack

Depend on your profile, if you are applying as a cadet you can ask for a date change with no problem. if you are trying to join as SFO or Captain you can ask, but you may lose the opportunity
