Here in Sweden we have many towers with just one position handling both the Approach and the Tower (and ground).
Most towers have PSR and SSR.

And no I dont find it difficult to switch between...

In Sweden you can buy a scanner and it is legal to listen to anything you like (police, the neighbours wireless telephones, ATC ). But it is illegal to tell anyone what you have heard.
