That’s pretty sad. Some engineers signed off the work as completed, when it obviously was not. Could have cost lives, as it was it did cost the price of all that fuel they dumped.

There was a "substantial shift" in N1 Vibs, #2 engine. Post landing inspection revealed a pretty good ice build-up on the engine spinner. Probably some ice broke off and was ingested.
