That would be the result of crews flying the wrong SIDs, leading to various exciting events. The procedure is cumbersome but so far it seems to work.

Courts have shown themselves time and again not to grasp fully the subtleties of ATC. Maybe there's more to the story but it seems to me that this is the daftest one ever - a trainee...

It's probably the main nightmare scenario for a tower controller, and where I work, it is dubiously referred to as a "piggy-back".

The conflict can become dangerous well below the missed approach...
