Yep, I got it WRONG about DCA. This is a fuller version of the incident as released by the FAA.

I am not sure that the word "standard" is correct for that kind of procedure. For ICAO the basic structure is One airspace,One controller and a lot of major european airports are still working like that.
One reason of implementing this devious way from ICAO is certainly the lack of radar rating given to "aerodrome controllers" (DOC4444)giving them the ability to move quickly aircraft in case of such incident, without any additional coordination.
And also the increasing step between surveillance tools quality available for aerodrome control service and the old fashionned "visual" ICAO philosophy..And finally, seen from the profit makers why to to put a radar qualified ATCO if you can pay less a basic aerodome controller...
