Blackhawk Drivers and Icing

To those who fly UH-60's...If you have encountered icing conditions in flight and activated the rotor ice protection system...
  1. How well did it work?
  2. Do you know of any instances where T/T blades were changed-out because of it didn't work?
  3. Why did the system not work?
  4. Do you know of any blades that were destroyed by water that somehow got inside the blade and then froze?
