S61 Incident

Hi,
Wandering if anyone can help. Does anyone know of a S61 incident where on shutting down the aircraft on an offshore platform the aircraft lost a main rotor blade due to spindal corrosion ? Possibly in the Norwegian sector a good while back ?

Can anyone shed anymore light ?

From: Beyond the black stump!
KLM machine in 1991. Slung a blade and sleeve on the heli-deck. Corrosion from inadequate sealing at overhaul. Aircraft recovered and flew again. No injuries.

Thanks, was there a picture posted somewhere of the a/c being recovered ? Maybe rotorheads around the world ?
