G-BJVX accident report available

The formal report into the S76 which shed a blade in the North Sea in 2002 is now available from the AAIB website.

11 deaths, all because a of manufacturing "anomaly", a lightning strike and a blade returned to service afterwards.

from the conclusions to the incident....

5 Almost immediately after the blade failed the centrifugal force imbalance
created by the missing blade section tore the main rotor gearbox from
the fuselage.

This is the part that shocked me !
