fish Cockpit window

is it true, that one of the most common reasons for an aborted takeoff is a cockpit window, which has not been closed/secured properly ?

"Cockpit Windows" suddenly appeared as an item in our B767 Before Start Checklist after a Management Pilot (not C & T) carried out a RTO when his window popped in and slid back.

For info, it is recommended not to exceed VREF+80 because of handcrank forces and may not be possible to open or close the window above approx 250 knots.
