Taking off without a DP or SID
Lets say you are released for departure from a satellite airport (airport in a metropolitan airport that is not the primary). You takeoff and try to contact departure but cant due to freq congestion. Are you cleared into the bravo? And even if you are able to promptly contact departure do you have to hear "cleared on course" before you can start your turn on course?

If you are released IFR, having received a proper clearance either by radio or telephone, no specific ATC clearance is required from the controling TRACON to enter Bravo airspace.
If you depart VFR from other than the primary airport, a specific prior approval must be obtained to enter Bravo airspace, IE: you cannot just barge right in, unannounced.
Turning on course would be at the pilots choice if IFR, having provided obstacle clearance (visually) preferred, if other instructions are not received to the contrary.
