Lost Private Licence

Hi
I have not flown privately for a couple of years, I am going to re-validate my rating in February, however I cannot find my PPL anywhere, does anyone know what form I need to fill in for a new one? I cannot find this anywhere.

Application for a replacement licence SRG 1117
You will also need to complete SRG 1104 as you will be issued with a new EASA licence

Furthermore, you will probably find a new license will not be issued without a valid class or type rating and a valid LPE.

I can only suggest you call the CAA and let them make a list of what you need and in what order.
