CAO 48.1 Table?

I find CAO 48.1 difficult to comprehend - don't laugh.
Years ago, there was a table/chart format that was extremely easy to follow. I believe the AFAP or Ansett had a similar one.
Does anyone know where I can obtain a copy of the simplified table/chart. Thanks.

AFAP members received a 2003 diary with the chart in the front pages.

This is not a wind up -- if you are a working pilot, why not join the AFAP, get the chart, the diary and full and free legals if you should ever need it
