Cotton Pilot Shirts
A friend of mine had a similar issue back a bit (probably 10-12 years ago now if I think about it... ). I understand he had some cotton business shirts tailored and the result was very good. I think all up (at the time) the finished shirt cost was only around $10-15 on top of "normal" epaulet shirts from "pilot" shops.

They do a great 55% cotton shirt at pilotshirts.com . Well designed pen and sunglasses slots in the pockets, nicely tapered at the waist too for those who dont fit into the 'parachute' cuts. I wear them in the tropics and they work a treat. Shame they dont ship outside the U.S. Maybe if enough foreigners hassle 'em....

My last holiday to the far east involved a stop at a tailors.
12 shirts 100% cotton (of the pilot variety...8 short sleeve, 4 long) all for about $90 USD.
They are still going strong 5 years later..and I'm tallish 6"4, about 110kg and I've always struggled to get shirts to fit me across my shoulders whilst not having a 19' neck!

I find Fairmark shirts are the best "off the rack" fit. Suitable length without looking like a parachute, although a poly/cotton blend.
