When should a captain be addressed as captain?

Ther is a controller still working, who used to be called commander. Apparantly he once applied for a bank loan and on the application form he was asked for his tittle, eg Mr, Docter etc, and one of the options was Commander, so he decided to be funny and ticked this box. Sure enough 2 days later the bank rings up the ATC centre to confirm that Commander Smith(not his real name) does indeed work there.
