A simple TOFO policy which was applied both in the mob and subsequently in a very large (huge!) global (US owned) corporation.
Claim for everything.
Complete all paperwork on the firm's time.
Make completion of claims a daily priority (phone off hook, door closed, do not disturb sign up).
If allowance does not meet expense...don't go (or use only service/company resources).
Never had a problem; never had a complaint.

Once worked with a french company on a job in a remote part of the third world - about 1400 guys on site
All expenses where available two days after the end of the month, cross referenced with the contract, originals attached and carefully broken down for our convenience
they had a couple of guys working full time generating them in advance in a hut round the back
over a beer a couple of years later the manager said the tough trick was to keep a tally of what they'd actually spent and what they'd submitted - he was quite shocked when I suggested they were fiddling "oh no, harry, we just made sure we got what we were entitled to"
