Livepool GLEX incident
Looks like it happened as the aircraft used a RET to exit the runway and perhaps the pilots were not paying 100% attention while taxiing off and hugged the right side of the taxiway instead of staying on the centre line.
But then who at the airport decided it was OK to finish off a taxiway in this matter... an accident waiting to happen?

Taxiway c/line could do with a lick of paint as well...perhaps a `no hard-shoulder in 20 yds`markers` as well....
