Slingsby Falcon ?
It is. Specifically a Falcon III modified as a flying boat for use by the Windermere branch of the ATC. Up to 2009 it survived in the Steam Boat Museum on Windermere but that closed " for refurbishment" in that year.

Very similar to the Fieseler Fi 156 Storch or the Antonov OKA-38

Something called Bisnovat Samolyot 5-1 (three gliding tests before crash). Or maybe 5-2 (16 gliding tests before abandoned. No powered flights).
