ATM concept plan

Is there anybody out there who has a template of an ATM Concept Plan other than ICAO Doc 9854? Was rather difficult to find anything on any website

Google "SesarJU", go to "SESAR JU Governance", click on the "Library" and go down the page to "Sesar Definition Phase" and select "D3 The ATM Target Concept". That will give you the ATM concept for Europe.
