NZ CAA recently published its report into an incident where a B767 "captured" a non-existent glide path. Flightdeck ILS indications, however, were substantially normal.

The report goes into a lot of useful background on ILS technology and some of the consequent operational problems which might arise if crews fail to play "devil's advocate" during critical stages of flight.
