WOW! that is a shame, coming over from the hanger so probably maintenance folks at the wheel. I've seen this type of accident before when MX had some circuit breakers pulled for hydraulics systems (brakes).

Probably a couple levels of supervision as well.

Mechanical or technical malfunction of towing equipment is out of the question, then?
