Just a question:

Is there an equivalent form in europe to a US/FAA NASA (ASRS form).

I didn't know if there is something like that in europe as well.

The UK equivalent is probably CHIRP (http://www.chirp.co.uk/) and you can also submit MORs (http://www.caa.co.uk/application.aspx?catid=33&pagetype=65&appid=11&mode=detail&id=214) confidentially.

Every country in the EU should have some way of reporting certain safety events (and it's quite a long list). Sadly although the EU said that these things should be reported, 'they' did not specify any common way of doing so.
