Just a question:

Is there an equivalent form in europe to a US/FAA NASA (ASRS form).

I didn't know if there is something like that in europe as well.

The UK equivalent is probably CHIRP and you can also submit MORs confidentially.

Every country in the EU should have some way of reporting certain safety events (and it's quite a long list). Sadly although the EU said that these things should be reported, 'they' did not specify any common way of doing so.
