Reference the 08R at YVR....from our documentation:

maintain 2000
shuttle climb 6000

I'd say that means maintain 2000 feet until in the hold, only then shuttle climb up to 6000. ...on the basis that it seems by defintion a shuttle climb/descent can only be done in a hold..
