If they break it at the very beginning, why don't you leave?
What are you, an idiot? In this field, you simply cannot change jobs at the drop of a hat. Let's say; you make the decision to leave. It will take months, at least, to obtain another job. Then there is the hassle of moving to another country (essentially, changing jobs in this profession means: Moving to another country), medical, training, validation, getting a house/car/life etc. It's not like quitting your job as a carpenter, and starting the next day with another builder.
On the other hand, the providers are like oil tankers- they take a long time to turn around. By the time they piss the staff off enough that they become short staffed (staff do leave over time, when pushed), maybe 18 months pass before they realise something has to be done, then the time taken to actually do it. It's a 2 to 5-year cycle. Lessons forgotten (and fairly recently so, in this case) have to be learned again. Maybe even learned the first time?
