A colleague had a class 2 renewal today here in LA, that was $100. A class 1 is a little more, $150 if I remember correctly. I imagine prices in LA are at least average.

From what I remember it was about £100 quid extra to do UK and FAA medical together in UK, but can't remember which way around it was, ie 100 to add a faa to a caa or 100 to add a caa onto a faa

I'm really glad that I did as I finally needed to get glasses for the class 2, which meant I could get that sorted out before I left the UK

As a matter of interest, where are you intending to fly in the US? and do you have all the CPL experience requirements already? It's worth an early meeting with the examiner to make sure they are clear about foreign logbooks, places and distances

I had an interesting debate with my school about the cpl long cross country, they claimed one of the legs had to be 250nm long, rather than one of the landing points being 250nm from the point of departure. This appeared to stem from the fact that most people end up back at base for obvious reasons and so examiners are used to checking the destination leg length. The DPE specifically mentioned this point when we were trying to book the check ride. Although the FAR 61.129 looks pretty clear to me, it took a call to the FSDO to clear it up to everyone elses satisfaction. That would have been bad news had it occured on the day of the checkride. I had similar issues on the private checkride.
