The ticked box by the examiner (not instructor) will be sufficient. I sent the originals of my forms and medical by recorded delivery, and all was good.
The CAA are very busy with EASA conversions at the moment, so you may be looking at a wait of a couple of months for the licence. They are apparently processing applications from September at the moment. As you're in Hampshire, your best bet may be to spend a day visiting Gatwick and you pick up a new EASA licence within a few hours. There is no extra cost, and you save £6 on the courier charge.
