SRG1102 is not required, as that is for UK CAA licences only - not JAA.
I made that mistake. It's not a problem if you do send that in, but it just saves you the time of filling it out. SRG1104 is needed to convert to EASA.
I'm not sure about FRTOL, as I was in a different position when I converted to EASA.
Make sure you have level 6 English proficiency, otherwise your application will be delayed until you obtain it.
