JAR PPL to EASA PPL Conversion.

Apologies if I'm covering old ground. I have a JAR PPL with a valid SEP and the license is required to be "re-issued not later than 10/12/12".
Im trying to find out what I need to do to get the license re-issued and at the same time converted to an EASA PPL, as I have had a few conflicting reports. I've also had no joy from the CAA.
I have a Form SRG 1157 (Skills Test) and a Form SRG 1107 (SEP Rating) from my last SEP renewal filled out and ready to send. As far as I can tell I attatch the Form SRG 1102 (Licence renewal) and Form SRG 1104 (EASA Conversion) to my test paperwork and send it off with £73, can anyone let me know if this is correct? Also do I need to apply for the FTROL separately or is this sent out at the same time as part of the conversion?
Also just to clarify do I need to have a specific aerobatics tick on my licence now or is my prior experience sufficient? I also want to make sure I don't need to do anything special to retain my VP Prop and Retractable Undercarriage difference training.
