Fatigue is one factor, MACHO factor is another reason.
I can do it, I can't be wrong, It's too embarrasing, those attitudes are a sure recipe for disaster.....
And the Capt was not the only one with those attitudes, the entire organisation is full of people with that mindset, that includes Pilots, trainers/examiners and the management.
A tragedy could have been avoided.........
Atleast, the current & next generation of pilots need to learn a thing or two from this accident......
