The point is that it is
not simple where there are obstacles. If you design a splay that will capture almost all of the points from which the missed approach will be commenced then it will be far from optimal for take-off performance simply because it will be so much wider at the departure end than is actually required.
As a separate issue, it would be nice to use the RTOW as the RLW but a few issues get in the way, such as MLW and the various approach and landing climb gradients (being much more substantial than take-off gradients.

While I appreciate that Pixie may consider them to be VFR destinations, the weather is irrelevant to the extent that the climb capability OEI does not vary with visibility or cloudbase!!!! Hagen is less of a problem than Goroka - outclimbing the pine trees is only the first problem.
