Question ICAO holding patterns vs FAA's

Could anyone tell me exactly how ICAO holds differ from the FAA's, if at all? Are the orientation procedures the same, i.e., do they use a cardinal hold direction in each clearance along with an assigned radial or mag bearing from the station? I know the double-up is only necessary for intersection holds, but the FAA issues a cardinal hold direction with station holds, too, probably for conformity.
