In my own poor Rigger's opinion, the most aerodynamic change made on the 787 that I can see is the CFR manufacturing system which makes smoother outside surfaces that, in turn helps smoother airflows.
Oh yes...and the nice crinkley bits on the rear of the engine nacelles - to mix hot & cold gasses to reduce noise, apparently.
