FAA to DGCA conversion forms and questions

Hey guys

I am about to submit my docs for the conversion and I have a few questions.

1) I have an Indian RT permit and I was searching for the application form for the DGCA FROTL. Can anyone guide me to the FROTL application form

2)In the application form do I need to fill up the technical specific details ? I know that DGCA takes only the planes you flew for the gft's as the ones you are endorsed on. For example I got my GFT day/night/ir on the C172 and the Pa34 respectively. Do I need to put C172/Pa34 or just leave these areas blank ?

3)Is there any dgca official (rather than a clerk) I can talk with in the office to make sure my application is completed?

4)I am submitting my FAA logbook as I have not made a separate indian logbook. Naturally there are a few whiteners for mistakes. I have asked my CFI in the school to put the school seal next to these edited entries. Will the dgca create a fuss over this ?
