Corel Draw 10 (I don't know if 9 has the option as well) does the job very easily. Base is a 150x150 pixel picture, saved as gif, than saved as icon, resizes automatically to correct dimensions.
If anyone wants the above icon than
right click and save target as and select where you want to save it.
