The worm you're alluding to has 2 other modes of transmission, via network drives and USB devices. Once on a network it also has a mechanism for scanning for weak passwords. While the patch (MS08-067 IIRC) deals with the most egregious vulnerability, applying it doesn't protect you if someone uses a USB stick they've previously used in an infected home PC...
