The two runways in NRT are separated by a couple of kilometres, they are not side by side and have diametrically opposed taxi routes. You cannot mistake one for the other. You cannot taxi for one and mistakenly end up on another. Departures on each runway are strictly controlled by a slot system, of which ATC are very aware.
For a crew to take off on the wrong runway there, ATC also would have to have screwed up mightily.
