Off the top of my head...
ICAO licences are perpetual
but individual nations may impose limits. Eg the UK CAA JAR ATPL requires renewing every 5 years. (listen to the cash register)
Probably will need to do a full type rating on the B737.
Definitely will need to do a medical.
