Off the top of my head...

ICAO licences are perpetual but individual nations may impose limits. Eg the UK CAA JAR ATPL requires renewing every 5 years. (listen to the cash register)

Probably will need to do a full type rating on the B737.

Definitely will need to do a medical.

The best I can do at the moment is from the UK CAA "LASORS".

Section G.
Scroll through to Section 5, Expired Licences.

Now, for the specifics about the type rating.
See Section F F4.4

This is for a UK JAR ATPL. I suspect other JAA countries will have similar requirements.

For the JAA source, JAR FCL-1
Within the document search for the words "expired" etc.
