The bulges in the doors were to allow the stretcher/extended seat to fit in. We did have the capability for external stretcher pods, (as seen on M*A*S*H), but casevac was fairly rare. I can assure you that to be in an external pod is not a pleasant experience!
Out of interest, those with keen eyesight may see a white line down the port side of the cockpit area.
