Shoes with extended heals can be very dangerous in certain A/C. It is possible to have the extended heal jammed between the rudder pedal (dead/dying engine) and the cockpit floor.
A Learjet crashed after EFTO from Amman, Jordan years ago. The handling pilot was wearing cowboy boots and the heel became jammed between the rearward moving rudder pedal and the floor. IIRC.
It is for this reason I will not allow a F/O to wear high healed shoes/boots in an A/C that I am the PIC.
