I'm also a bit confused by the application.

I hold an FAA ATP.

Obviously an FAA certificate IS an ICAO license. And I had to pass a ground (written) examination. So should I circle "All ATPL subjects passed"? I feel that if I don't circle it I won't meet the screening criteria. But of course in the US we don't have "subjects". It's only one test.

Someone wrote that for "Multiengine instrument rating" I should put the date of my next air carrier required flight check. But that seems not right to me. For one thing, it has no bearing on my certificate or right to exercise "multiengine instrument" priviliges. I'm planning on just putting "no expiration date".

Additional question:

The application says to provide a "passport style" photograph of yourself. Curious if people here actually provided a little 2"x2" photo or included a normal size photo in the passport pose.
