Sections 3.4, 3.5 and 3.6 contain the basic requirements. It's up to the operator, often in conjunction with the OEM, to develop compliant procedures and processes. There's a lot of ETOPS experience kicking around this country.
Training has a lot to do with procedural matters, and so includes Dispatchers as well as pilots.
Integration of the Maintenance Control function with Operational Control one of the things TC looks for.
Westjet originally limited the ETOPS pilot cadre to those with previous experience from other companies (eg, Canada 3000). I believe that as (if) their overseas operations expand, they'll eventually need to qualify lots more folks, and perhaps everyone. (At ZOOM, Harmony and Air Transat, all pilots would need the training. For Air Canada and Skyservice, all pilots on the applicable equipment B767, B757, A330 would be trained)
