Has there ever been a case or does it occur where a person who only did thier training on the 300/400 then jump into a NG without ever really knowing the differences?

No in short is the answer for example pilots who got jobs with DJ with extensive 737 EFIS time had to do a short differences course!
And vice versa people who have only flown the NG rare cases going back to the EFIS have to do a differences course even though the CASA rating is for a "737-300/800"
