You generally need to have a medical in the country that you are doing the training. However some countries will 'validate' a medical from another countries. I'm pretty sure the US will issue an FAA class 1 medical on the basis of a UK class 1 etc. Am not 100% rho. The best bet is to ask the authority of the country that you wish to train in.
