If as supposed above, the crew were startled, unable to comprehend, then why, with hindsight, should we expect CRM to be effective.
The industry increasingly promotes the idea that safety interventions will be effective in all situations, all of the time.
We forget that the real world is uncertain, and that whilst "
the human element is the most flexible, adaptable and valuable part of the aviation system, it is also the most vulnerable to influences which can adversely affect its performance" ICAO Human Factors Training Manual.
Time to revisit the assumptions in safety management and operations.
