The upgrade issue is very precarious
If you get upgraded by ground staff it's usually due to oversales in economy cabins (or very kind staff)
If you get upgraded by the crew on board it's usually an observant senior crew member who's spotted crew on the pax list
(make a note of their names and be sure to return the favour.)
There always used to be an unwritten law that crew upgraded staff whatever ticket they were on (provided they were dressed appropriately of course)
Can we bring that back please, I know I've always done my best to do this.
(Crew really don't mind if there's not enough catering, but the chance for a good night's sleep is priceless.)
