Where, pray tell, is the oversight and input from the Licencing Authority i.e CASA?

Is this not their Obligation, Duty of Care and Responsibility?

Agree with all above, obligation is to their shareholders, duty of care is a foreign concept in Qantas , and responsibility is to ensure that they get as fatter bonuses as possible.
